Iowa Tax Form Error

A clerical error has Iowa officials resending thousands of tax forms last week, the state mailed out $15 thousand 10-99 forms with the wrong tax identification number.

Those affected are business owners who provide medicaid.

An agency spokesman said the error happened during data sorting in the print to mail process.

State officials aren't yet saying how much it'll cost to fix the mistake.
